COVID-19 Student Vaccination Requirements (8/4/2021)

The University of Maine at Farmington requires the submission of official immunization records prior to attendance using the Immunization Form. This applies to all international students, as well as scholars, who will live in a campus residence hall.

Required Immunizations

Maine State Law requires all degree-seeking students and full-time, non-degree students born after December 31, 1956, to provide the following proof of immunization:

  • One (1) dose of Tetanus/Diphtheria (Td or Tdap) received within the last ten (10) years.
  • Two (2) doses of Measles (Rubeola), Mumps, and Rubella (German Measles) received after (not on) the first birthday.  If measles immunization was done prior to 1968, you must provide proof it was done with a live virus.

*Titre tests with positive results for immunity to Measles, Mumps, and Rubella are accepted as long as they are signed or stamped and clearly indicate immunity to all three diseases.

Official Immunization Records

Immunization Form must indicate the date of immunization and is REQUIRED to be signed and stamped by a Healthcare Provider. Students should email the completed Immunization Form to Lynne Eustis at leustis@maine.edu.

Recommended Immunizations

Visit this UMF Student Health Center website for a list of recommended immunizations. International students who have received one or more of the recommended immunizations should email documentation to Lynne Eustis at leustis@maine.edu.
